Telehealth uses electronic communications to allow a clinician and patient in different locations to exchange health information and provide care. By participating, a patient acknowledges the nature of telehealth and its benefits, alternatives, and limitations.
Potential benefits and risks
Potential benefits include improved access and convenience. Risks include technology failure, incomplete information, privacy or security events, and the inability to perform an in-person physical examination. Evertitan provides care exclusively through telehealth. When an in-person examination or service is necessary, the patient may be directed to an appropriate local healthcare provider. Telehealth may not be appropriate for every condition.
Patient acknowledgments

Patients may be given treatment alternatives and information about material risks and benefits. If a compounded medication is considered, patients should understand that compounded drugs are not FDA-approved and the FDA does not verify their safety, effectiveness, or quality before marketing. Availability may vary by jurisdiction, pharmacy, and clinical circumstances.
Medications offered through Evertitan are dispensed exclusively by independent, appropriately licensed compounding pharmacies in Evertitan's designated pharmacy network. The dispensing pharmacy may vary based on the medication, patient location, availability, clinical circumstances, and applicable law.
Evertitan does not own a pharmacy or sell prescription drugs directly. Pharmacy fulfillment, shipping, counseling, returns, and related charges are governed by the dispensing pharmacy's policies. Evertitan does not issue a prescription before the required physician evaluation.
Testosterone is a Schedule III controlled substance. Controlled medications are prescribed only when clinically appropriate and only when the treating practitioner maintains all required state and federal authority, including applicable DEA registration requirements, for the patient's location. If those requirements are not satisfied in a jurisdiction, controlled-substance treatment is not offered there.
Financial, Cancellation, and Refund Notice
Pricing for the requested medical service, cancellation terms, refund eligibility, recurring-payment terms, and any known laboratory or pharmacy charges are presented before the patient authorizes payment. Medical services already rendered are generally not refundable. Medication and laboratory charges may be billed separately and may be subject to the dispensing pharmacy's or laboratory's policies and applicable law.
No recurring charge is initiated without clear advance disclosure and authorization. A patient may cancel future recurring charges according to the terms presented at enrollment. Cancellation of a recurring plan does not automatically cancel a prescription already transmitted, medication already dispensed, laboratory work already performed, or medical services already rendered.

If you expressly opt in, Evertitan may send appointment, account, care-coordination, or other service-related text messages. Marketing texts, if any, require the consent applicable to that message type. Consent is not a condition of purchasing services. Message frequency varies; message and data rates may apply.
Reply STOP to opt out or HELP for help. You may also call (385) 479-7575 for support. Opting out of texts does not prevent necessary communications through other permitted channels.
